Practice Policies
Appointments
• Appointments are required but we will do our best to accommodate any acute or urgent problems. These will be treated at the discretion of the Practice.
• If you are 15 minutes late for your appointment you may be rescheduled.
• Any patient who fails to arrive for a scheduled appointment without canceling less than 24 hours prior to the scheduled time is considered a “no-show.” A patient who consistently fails to present themselves for scheduled appointments is considered a chronic no-show. A patient who is a no-show more than three times is dismissed from the Practice.
• Any patient who reschedules an appointment more than three times is also considered a no-show and will be dismissed from the practice.
Payment
It is the policy of the Practice to offer payment plans to assist patients who have difficulty paying outstanding balances because of financial hardship. We accept all major insurance providers. The remaining balance can be paid with cash, check, debit, Visa or MasterCard. The Practice offers a 15% discount to all self pay patients if the balance is paid in full at the time of visit. If a self pay patient is unable to pay in full at that time 25% of the visit must be paid. The remaining balance must be paid in full prior to next visit. A $50.00 Service charge will be applied to all returned checks. If a returned check fee is applied to your account, all future visits will require balance to be paid by cash or credit card only.
Forms To Be Completed
A $20.00 fee will be assessed for the completion of all forms which include: Disability, FMLA, Workers Compensation, Patient Assistance, etc. This fee must be paid prior to the completion of each form.
After Practice Business Hours
The Practice offers a voicemail service for detailed messages. These messages will be addressed by the staff of the Practice and you will be contacted by the next business day. If you should have an emergency or acute care problem that requires urgent treatment please contact the Physician on call at the hospital.
