  Practice Policies

Appointments
  • Appointments are required but we will do our best to accommodate any acute or urgent problems. These will be treated at the discretion of the Practice.

  • If you are 15 minutes late for your appointment you may be rescheduled.

  • Any patient who fails to arrive for a scheduled appointment without canceling less than 24 hours prior to the scheduled time is considered a “no-show.” A patient who consistently fails to present themselves for scheduled appointments is considered a chronic no-show. A patient who is a no-show more than three times is dismissed from the Practice.

  • Any patient who reschedules an appointment more than three times is also considered a no-show and will be dismissed from the practice.

  • If you are seeing the doctor and you have friends or family members with you and they need medical attention we will be happy to schedule them their own appointment. We cannot provide medical service to anyone unless they are registered as a patient on the schedule.

Payment
It is the policy of the Practice to offer payment plans to assist patients who have difficulty paying outstanding balances because of financial hardship. We accept all major insurance providers. The remaining balance can be paid with cash, check, debit, Visa or MasterCard. The Practice offers a 15% discount to all self pay patients if the balance is paid in full at the time of visit. If a self pay patient is unable to pay in full at that time 25% of the visit must be paid. The remaining balance must be paid in full prior to next visit. A $50.00 Service charge will be applied to all returned checks. If a returned check fee is applied to your account, all future visits will require balance to be paid by cash or credit card only.

Deliquent Accounts
An account is considered delinquent if no payment has been made on the account, or no payment plan has been structured for the account within 90 days after charges have been incurred or the account is in default under the terms of the payment plan.

If your account is delinquent, additional refills for medication and/or new medical services from this office will not be provided until the delinquent account is brought current. If your account is turned over to collections you will be dismissed from this practice and we will send your medical records to another physician by request. Our billing system links patient accounts who share a household. Therefore, if any account with whom you are linked is delinquent the delinquent account policy applies to your account and the policy will be enforced.

Forms To Be Completed
A $25.00 fee will be assessed for the completion of all forms and requested letters of dictation which includes but is not limited to disability, FMLA, Workers Compensation, Patient Assistance, etc. This fee must be paid prior to the completion of each form. For copies of medical records one free copy will be given, if needed, per year, any additional copies within that year will be .50 per page to be paid before you receive the copies.

After Practice Business Hours
The Practice offers a voicemail service for detailed messages. These messages will be addressed by the staff of the Practice and you will be contacted by the next business day. If you should have an emergency or acute care problem that requires urgent treatment please contact the Physician on call at the hospital or the emergency room.

Prescription Refills
Medication refills are generally prescribed for a period of time that should, in most cases, maintain your medical therapy until you next regular interval follow-up appointment. When you are out of medication, we advise that you call our office to determine if it is time for your follow-up. For all prescription refills, we may take up to 24 business hours after you call us to get the prescription called into the pharmacy or a written prescription for you to pick up, so please call BEFORE you run out of medication. If you have mail order prescriptions it is your responsibility to mail your prescriptions.

Insurance Coverage
We strongly recommend that you contact your insurance company prior to being seen in our office to determine your health insurance benefits. Some visits and/or procedures performed in the office may not be covered by your insurance, such as preventative visits, visits for medical certificates such as DOT or sports physicals, etc. Also, we initially send all blood work done in our office to Trigg County Hospital unless you state that you would like it sent somewhere else. Any procedures, tests, labs, etc. are your financial responsibility, if not covered by your insurance. It is your responsibility to know what coverage you have with your insurance company.

Medication
Patients that are prescribed controlled substances such as chronic pain medication, ADHD medication, insomnia medication, anxiety medication, or narcotic medications will be receiving a controlled substance policy to read and sign. We do not supply a 90 day prescription or mail order prescription for controlled substances.
